Please note that there are 2 different forms depending on age (Preschool and K-12)

Preschool: The medical form must be turned in by the first day of school and annually by the 1st day of the new school year.

K-12 (not for online-only): NC Health Assessment Transmittal Form. The medical form must be turned in by the first day of school.

*Please note that we can not accept digital forms. Each form needs to have the physicians signature on it - our forms here are recommended.  A medical action plan must be on file if your child has a diagnosed medical condition, and symptoms could occur during the school day, and/or your child needs to have medication on site. Your child may not be able to attend school without an action plan and the medication (if applicable) on site. Medical action plans have to be completed by your child’s doctor. Medication (if applicable) will have to stay at school, must match the medication mentioned on the action plan, must be unopened and be in the original packaging, must have the pharmacy sticker attached with your child’s name, and must be unexpired. Please note that, depending on age group and/or type of action plan, the action plan might only be valid for 6 months and have to be renewed during the school year. 

PLEASE NOTE, ALL ACTION PLANS MUST HAVE THE DOCTOR'S SIGNATURE.
